Patient Safety Center of Inquiry: Measurement to Advance Patient Safety (PSCI-MAPS)

Funded since 2012, the objectives of the PSCI-MAPS aim to advance the field of patient safety measurement and develop tools that can be disseminated and implemented throughout the VA healthcare system. Our multidisciplinary team includes highly experienced experts in the fields of patient safety, clinical care, infectious disease, health services research, and healthcare informatics. The overall goals of the PSCI are to improve measurement of patient safety by facilitating better integration across existing patient safety measures, and developing and testing new measures that improve on traditional approaches. Guided by an Advisory Board of four VA and four non-VA leading patient safety experts, as well as a number of VA operational partners, including the VA Office of Informatics and Analytics (OIA) and the VA Health Information Management Systems (HIMS), the Center is actively involved in four separate projects. These projects utilize existing patient safety measures that are generated through multiple data sources: administrative data, electronic medical record review, incident reports, and the VA Surgical Quality Improvement Program (VASQIP).  Our work in current and past PSCI-MAPS projects provides a comprehensive picture of patient safety in the VA that facilities can use to effectively target areas for quality improvement and promote safe practices through well-designed interventions. The PSCI-MAPS is funded through 2018.
